Surveys

Two iPads are being raffled off, one for each of the below surveys. To be eligible for the raffles, you must be a registered attendee of the symposium and not an employee of any of the exhibitors or sponsors. For raffle eligibility, the marketing survey may be filled out any time until 4:15pm ET the day of the symposium, and the course evaluation may be filled out any time after the symposium until Wednesday Sept 21, 2011 at 12 pm ET. You only need to identify yourself if you are entering a raffle. Even if you are not eligible for the raffles, you are encouraged to fill out the surveys anyway if you attended.
